When calling for an appointment please have your insurance card ready. Please bring any prior imaging and pertinent medical records including an updated medications list with you along with your insurance card and driver’s license. Minor’s must have a parent or guardian present, or have a Consent for Treatment form on file.
Please arrive 20 minutes early to fill out new patient forms, or print them below to fill out ahead of time. You can bring these forms with you at the time of your appointment or fax or email them in advance.

We are dedicated to providing the best quality care to all of our patients, and want you to feel comfortable discussing your medical concerns. All of our providers are happy to work with a translator when needed. We have medical assistants available to translate in Spanish, or if you prefer a Spanish speaking provider, please ask for Angie Gonzales, PA-C.
